Parents should read, understand, and sign this form prior to their children participating in our league. Any parent guilty of improper conduct at any game or practice will be asked to leave the sports facility and will be suspended from the following game. Repeat violations may cause a multiple game suspension, or the season forfeiture of the privilege of attending all games. PreambleThe essential elements of character-building and ethics in sports are embodied in the concept of sportsmanship and six core principles: Trustworthiness Respect Responsibility Fairness Caring Good Citizenship The highest potential of sports is achieved when competition reflects these “six pillars of character.” I therefore agree: I will not force my child to participate in sports. I will remember that children participate to have fun and that the game is for youth, not adults. I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others. I will learn the rules of the game and the policies of the league. I (and my guests) will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, officials, and spectators at every game, practice or other sporting event. I (and my guests) will not engage in any kind of unsportsmanlike conduct with any official, coach, player, or parent such as booing and taunting, refusing to shake hands, or using profane language or gestures. I will not encourage any behaviors or practices that would endanger the health and well-being of the athletes. I will teach my child to play by the rules and to resolve conflicts without resorting to hostility or violence. I will demand that my child treat other players, coaches, officials, and spectators with respect regardless of race, creed, color, sex, or ability. I will teach my child that doing one’s best is more important than winning, so that my child will never feel defeated by the outcome of a game or his / her performance. I will praise my child for competing fairly and trying hard, and make my child feel like a winner every time. I will never ridicule or yell at my child or other participants for making a mistake or losing a competition. I will emphasize skill development at practices and how they benefit my child over winning. I will also de-emphasize games and competition in the lower age groups. I will promote emotional and physical well-being of the athletes ahead of any personal desire I have for my child to win. I will respect the officials and their authority during games and will never question, discuss, or confront officials at the game field, and will take time to speak with coaches at an agreed upon time and place. I will demand a sports environment for my child that is free from drugs, tobacco, and alcohol and I will refrain from their use at all sports events. I will refrain from coaching my child or other players during games and practices, unless I am one of the official coaches of the team. OTHER RULES & REGULATIONSI have read, understand, and agree to the following. *We, the Giles County Softball League, have implemented the following Rules & Regulations: Refund Policy: Giles County Softball League has no refund policy. Photograph Release Policy: I / We, the parents / guardians, hereby give my / our approval to allow photographs of my child to be posted to Giles County Softball League's website and / or social media sites. I / We also give my / our permission to use the name and / or photographs of my child on any community or Little League International media, such as but not limited to newspapers, websites, and radio. JEWELRY POLICYI have read, understand, and agree to the following. *All items of jewelry including, but not limited to, earrings, rings, belly rings, bracelets, watches, necklaces, barrettes, bobby pins, and hair beads are forbidden and must be removed. Using tape to cover jewelry is not permitted. The players must be inspected by the coach before the start of the match. If a player is wearing or using unauthorized / dangerous equipment or jewelry, the safety manager or umpire must order the player to remove the item(s). If the jewelry in question cannot be removed, the player will not be allowed to participate in that game. The only exception to this rule is for medical alert necklaces / bracelets. In the event the player has a medical alert bracelet / necklace, the player must tape the item down to their body. LIABILITY RELEASEI have read, understand, and agree to the following. *In consideration of being allowed to participate in any way in Giles County Softball League games, events, activities, etc., I acknowledge and agree that: I understand that softball requires strenuous physical exertion and tasks, and I am fully aware of the risks and hazards that are inherent, including, but not limited to: physical illness, injury, death, or other consequences arising from my child’s participation in the activities. I take full responsibility for my child’s health and well-being on a voluntary basis and for any risks associated with engaging in a physical activity. I agree to abide by the rules and regulations of the Giles County Softball League and by voluntarily allowing my child to participate, represent that my child will also abide by them. I shall withdraw my child’s participation if I am advised by my doctor or authorized medical representative to do so. I expressly waive, release, and discharge Giles County Softball League and its coaches, employees, representatives, advisors, volunteers, assigns, and other affiliates, and their respective officers (the “Releases”) of any and all responsibility, los, harm, or injury in connection with softball activities. I accept that I would hold harmless and indemnify the Releases for any and all responsibility to any third party arising from my child’s involvement in softball for any property damage, loss, or personal injury. I accept all terms listed in this Waiver of Liability, fully understand its terms and that, by signing it, I have given up substantial rights, and sign it freely and voluntarily. Terms & Conditions * I / We, the parents / guardians of the above-named candidate for a position on a softball team, hereby give my / our approval to participate in any and all softball activities, including transportation to and from the activities. I / We know that participation in softball may result in serious injuries and protective equipment does not prevent all injuries to players, and do hereby waive, release, absolve, indemnify, and agree to hold harmless the local Softball organization, the organizers, sponsors, supervisors, participants, and persons transporting my / our child to and from activities and any claim arising out of any injury to my / our child whether the result of negligence or for any other cause. If applicable, I / We agree to return upon request the uniform and other equipment issues to my / our child in as good conditions as when received except for normal wear and tear. I / We agree to provide proof of legal residence or school enrollment and age. I / We understand that our child (candidate) must be eligible under the residence / school attendance and age regulations to participate in softball, and that if any controversy arises regarding residence / school attendance and / or age, the decision of the Giles County Softball League shall be final and binding. I / We further understand that if any participant on a softball team does not qualify for participation in the league based on residence and / or age, such participant and / or team on which he / she participates be found ineligible, and forfeit(s) and / or suspension of Tournament privileges may be decreed by action of Giles County Softball League. I / We agree that our child (candidate) may be required to try out for a team. If such does not attend at least 50 percent of the tryouts, local Board-of-Directors' approval is required for such candidate to be placed on a team. If applicable, I / We understand that our child (candidate) may be chosen at any time to play on a Major Division team, if he or she is of the correct age for such division as determined by local league. Declining to move up to such Major Division team will result in forfeiture of eligibility for the Major Division for the current season and may be subject to further restrictions by the local league. I / We will furnish a certified birth certificate of the above-named candidate to softball Officials. I / We understand that my information as the parent or guardian of such above-named candidate is kept by Giles County Softball League each year. You may opt-out of communications from Little League International at any time. ALL-STAR SELECTIONI acknowledge that I will be responsible for the cost of the uniform should my child be selected to an All-Star team at the conclusion of the regular season. If I do not wish to be responsible for the cost, I will exclude my child from consideration for selection to an All-Star team by informing our manager or board member.
